The Someone to Trust Advance Care Planning initiative is a comprehensive program to increase the number of Chicago area residents who plan ahead for their healthcare. Using advance care planning, individuals appoint an agent (healthcare power of attorney) and participate in meaningful conversations with their agent and healthcare provider. As a result, Chicago residents can control their healthcare should they no longer be able to speak for themselves.

A successful, metropolitan-wide, integrated advance care planning program will reduce the frequency of unwanted medical treatments, facilitate the delivery of quality, patient-centered care and ensure the wishes of patients are honored by the healthcare system.

Someone to Trust is an independent program of the Institute of Medicine of Chicago.
